Guideline for Level of Care Assessment

These are general guidelines used by the Care Planning Team to determine the appropriate level of care for an individual. A deficit in any area requires engagement in service planning. Please contact the Health Coordinator at 509-8270, ext. 155 to schedule an appointment for service planning.

Cottages, Villa Court and Mews

No personal care services (dressing, bathing, hygiene support, walking, transfers, appointment scheduling, housekeeping, shopping, meal preparation, laundry, or medication assistance), health monitoring, cognitive support or other types of supervision are included in the monthly maintenance fees. This type of support may be arranged in conjunction with the service planning through the Health Coordinator. Services may be contracted through Homestead Village Home Care Services, LLC or other home care agencies in conjunction with the service planning.

Residents must be able to transfer independently (move from bed to chair to commode) and vacate the unit in the event of an emergency, or they must employ approved staff to assist with these tasks.

Apartments / Supportive Services / Licensed Personal Care Home

No personal care services (dressing, bathing, hygiene support, walking, transfers, appointment scheduling, laundry, shopping or medication assistance), health monitoring, cognitive support or other types of supervision are included in the monthly maintenance fees. This type of support may be arranged in conjunction with the service planning through the Health Coordinator by enrolling in the Supportive Services program for an additional monthly fee. To provide resident choice and safety, Supportive Services, a licensed personal care home service, is provided to apartment residents who need assistance with one or more activities of daily living or instrumental activities of daily living, for an additional daily fee.

Activities of daily living include meal preparation, ambulating (getting from one place to another), toileting, bladder and bowel management, personal hygiene (bathing/dressing), securing health care, managing health care, and medication administration.

Instrumental activities of daily living include the following activities when done on behalf of a resident: doing laundry, shopping, securing and using transportation, using a telephone, making and keeping appointments, caring for personal possessions, writing correspondence, reading mail, engaging in social and leisure activities, using a prosthetic device, obtaining and keeping clean, seasonal clothing

Residents must be able to transfer independently (move from bed to chair to commode) and vacate the building via designated hallway routes within five (5) minutes, in the event of an emergency and/or during routine safety drills held monthly, or they must employee approved staff to assist with these tasks.

Gelhard House at Westvue / Memory Support in a Licensed Personal Care Home

Memory support and personal care services provided are:

  • Memory support and life enhancement activities for the memory impaired
  • Assistance with showering/bathing, dressing, ambulation, incontinence management and medication management
  • Meal preparation, laundry, housekeeping and shopping
  • Appointment scheduling, health monitoring and cognitive support
  • 24-hour oversight/supervision and safety monitoring

Residents must be able to transfer independently (move from bed to chair to commode) and vacate the building (including descend a flight of stairs in their exit path) within five minutes in the event of an emergency and/or during routine safety drills held monthly, or they must employee approved staff to assist with these tasks.

Apostles Center For Care / Skilled Nursing Facility

Provides extensive assistance and/or daily oversight of health status and is supervised by registered nurses. Services include:

  • Speech, physical and/or occupational therapy; skilled nursing services
  • Moderate to total assistance with showering/bathing, dressing, incontinence management, nutritional intake and medication management
  • Assistance with ambulation and transfers
  • Meal preparation, laundry, housekeeping
  • Appointment scheduling, health monitoring, cognitive support
  • 24-hour oversight/supervision and safety monitoring

Staff provide for the safe transfer/evacuation of residents in the event of an emergency and/or during routine safety drills held monthly.
